INSURANCE & PAYMENT
We are no longer in-network with any insurance providers.
Payment is due at the time of service, and we accept self-pay patients. Upon request, we are happy to provide a superbill that you can submit to your insurance company for possible out-of-network reimbursement.
Because every insurance plan is different, we recommend contacting your insurance carrier directly to determine your out-of-network physical therapy benefits and reimbursement eligibility.
